By accessing our website or using our services, you acknowledge and agree to comply with these Terms & Conditions and all applicable laws and regulations. If you do not agree, please discontinue using our website and services.
Services Provided
SLB Diagnostic Solutions LLC provides the following services:

DOT & Non-DOT drug testing

Hair, nail, oral fluid, and urine drug testing

Breath alcohol testing
-
DNA collection services
-
Rapid medical testing (CLIA-waived)

Mobile on-site testing
-
Employer & TPA services
-
Compliance and consortium enrollment
All services are subject to availability and may be updated or modified at any time.
Testing Limitations
We do not provide medical diagnosis, treatment, or interpretation of laboratory results. Test results are provided by certified laboratories or testing devices, and customers should consult licensed medical providers or legal professionals for interpretation of results where applicable.
Appointments & Scheduling

Appointments may be booked online, by phone, or in person.

Walk-ins are accepted based on availability.

Mobile appointments must be scheduled in advance and confirmed.

Late arrivals may be rescheduled at the Company’s discretion.

Emergency or after-hours services may include additional fees.
Payments & Pricing

Pricing is listed on our website and may change without prior notice.

All services must be paid at the time services are rendered unless approved otherwise.

Insurance, Medicaid, and Medicare are accepted for eligible services only.

Mobile service fees, after-hours charges, and emergency service fees are separate from test pricing.

Refunds are not provided once services have been rendered.
Identity & Documentation
Clients must present valid photo identification for testing. DNA clients must provide additional documentation as required by partner laboratories.
Confidentiality & Privacy
All personal and medical information is handled according to HIPAA principles and applicable laws. Please refer to our Privacy Policy for more information.
Results & Liability
SLB Diagnostic Solutions LLC is not responsible for:

Laboratory delays

Specimen integrity issues caused by client actions

Errors originating from partner labs
-
Misuse or misinterpretation of results

Employment or legal decisions based on test results
Mobile Services
Mobile services may be rescheduled due to:

Weather conditions
-
Traffic delays
-
Access issues at testing locations
-
Client unavailability
Fees may still apply in cases of cancellation or no-show.
Third-Party Laboratories
Some testing services are performed through partner laboratories. SLB Diagnostic Solutions LLC is not responsible for laboratory policies, processing times, or errors outside of specimen collection.
